I, the undersigned, agree to keep my dog under full control at all times while in class or in the training facility. I agree to accept full responsibility of my dog’s actions and will not hold Crazy Tails, it’s instructors or owner of the property we are leasing liable for any damage done by or to my dog, or any illness contracted. I understand that I have two weeks from the start date of class to withdraw with a refund of 25%. Refunds will no longer be given if I choose to withdraw from class after two weeks.
